REIMBURSEMENT Instructions

These are instructions on how to get reimbursed if you have been promised travel support for this meeting. All reimbursements will take place after the meeting is over. Once you have submitted all necessary documents, your request will be processed by Vanderbilt University, and you will recieve a check directly from them in US dollars. This will take 6 to 8 weeks.

To receive reimbursement, you must be either a US citizen or permanent resident, and if not, hold one of the following types of visas:

1) If you live in a visa waiver country, you can come on a WB or WT visa. To use this program, you should fill out the forms on the ESTA site. You will need to print out these forms to travel, and we will need a copy for reimbursement purposes.

2) If you are coming from a non-waiver country, you need to get either a B1, B2, F1, or J1 visa. If you come with an F1 or J1 visa, we will also need a signed letter on university letterhead from your university giving you permission to receive travel reimbursement from us.

3) If you are coming from outside the US, you need to fill out the Vanderbilt Business Visitor Questionnaire. Say you are visiting Vanderbilt. Ignore requests for uploading documentation. This should be done before coming to the meeting.

4) All participants receiving reimbursement must fill out one of the following forms: W9 form if you have a social security number or W8-BEN form otherwise.

5) For ticket reimbursement, we need a copy of your electronic ticket showing itinerary, cost, and method of payment; or if you have a paper ticket, the original ticket stubs with the same information.

6) For hotel reimbursement, we need a copy of the itemized hotel bill.

7) If you come from overseas, we will need a) a copy of the front page of your passport with your photo, b) a copy of your visa or visa waiver stamp, and c) the ESTA form if you use the waiver program.
