Those of you who have been promised financial support for travel related expenses, please fill out the UCSB Mathematics Department, Travel Expense Reimbursement Worksheet (up to the dotted line) and a UC Statement of Citizenship Status form, if applicable. You will get these forms from us. We need all original receipts of any expenses that you claim. If you can't leave the receipts with us before you leave, please send them to

If you are not a US citizen and not a resident, we need a photocopy of your passport (including picture) and your I-94. If you are a resident, we need a photocopy of your green card. If you are an employee of the University of California, it would be very helpful to have your UC employee ID number.
